  • disqualify Dean Blake for 5 years from holding or obtaining any type of operator鈥檚 licence in any traffic area
  • disqualify Dean Blake for 5 years from being the director of any company holding or obtaining an operator鈥檚 licence
  • disqualify Adam Philip Hayes for 5 years until 00:01 on 11 May 2023 from acting as transport manager under any operator鈥檚 licence
